       MISSION STATEMENT :
           Continuous quality improvement is necessary to ensure the accuracy of healthcare documentation.

       OVERALL TRANSCRIPTION GOAL :
           To accurately and efficiently transform the spoken voice into an accurate, grammatically correct, appropriately            organized, medical legal document.

       FOLLOWING MEASURES ARE TAKEN TO ACHIEVE ABOVE TRANSCRIPTION GOAL :

      1.   Accuracy improvement target will be set for each employee in conjunction with his or her immediate supervisor.

      2.   Each MT will work towards established performance standard; i.e., percentage of jobs sent for forced             proofreading, overall accuracy score, etc.

      3.   The quality analyst (QA) staff will give the daily feedback to the MTs about their performance.

      4.   The quality control (QC) staff gives daily feedback to QAs, i.e. accuracy, performance, line-count, etc.

      5.   The team leader will assess each medical transcriptionist in the work group to set individual monthly goals for             accuracy and productivity.

      6.   Daily feedback regarding accuracy rate and specific transcription errors are shared with individual             transcriptionist so that they may chart their progress towards the desired goal.

      7.   Question or concern regarding this daily reports may be resolved by the team leader and the Director of             Professional Practices.

      8.   Monthly reports are generated to summarize and quantify the information, which is collected daily.

      9.   New employees are proofread 100% until their team leader determines that they are producing correctly             formatted work at 98% or higher accuracy.

     10.  Failure to achieve 97% accuracy for two consecutive pay periods may result in the MT being put on probation.             Failure to achieve 98% accuracy by the end of the period of probation may result in dismissal.

     11.  The AAMT Book of Style will be used as a reference guide for formats and style questions. Each MT is required             to refer this book and other specified reference books depending on account assignment. Examples include:             Stedman's Medical Equipment Words, Stedman's Radiology and Oncology Words, Stedman's Abbreviations,             Stedman's Surgery Words, Stedman's Medical & Surgical Words, Stedman's Pathology & Lab Medicine             Words.

     12.  The current pharmaceutical reference (drug), spell check program, grammar check program, instant text are             included on the Beta Software workstations.

      CARDINAL RULES :
          Leaving a blank may be the most professional action a transcriptionist can take when dictation is inaudible or           unable to be understood.
